Toothbrush Abrasion
I see many patients that have "toothbrush abrasion". This is what it looks like:

Let's look at the anatomy of a tooth!

You'll notice that the outer part of tooth is the "enamel". The enamel is hard and white, and it covers the dentin. Dentin, unlike enamel is soft and yellow. Enamel varies in thickness and gets thinner closer to the gums.
When people brush their teeth with a lot of pressure, or with a hard toothbrush, they start to irritate their gums. This causes the gums to pull away from the tooth and expose roots. Roots are not covered with enamel. The individual will become more sensitive to hot and cold, and roots are more vulnerable to cavities.
Over time, aggressively brushing your teeth can wear away the enamel on the tooth. Since the enamel layer is thinner closer to the gums that's the first part to go. If this happens notches will be made through the dentin. Again, this increases sensitivity and makes the tooth weaker.
Many Punjabis have been led to believe that brushing with a lot of pressure will get their teeth "cleaner". This isn't true. Also, many will only use a brush with hard bristles because they don't think a soft one will get their teeth clean. Again... not true.
You can compare brushing your teeth to washing your car. You'd want to use something soft and you wouldn't want to scrub really hard. Most people don't want to scratch the paint on their car. So why do that to your teeth?
When brushing your teeth, don't apply pressure towards your gums, use circular motions (not back and forth), and use a soft brush.

Thanks for the information Dr. Kooner. Your knowledge is greatly appreciated.

How long have you had the white spot? It might be a fistula from an infection. Is there pus too?

what's a "pus"
nah just a few days.
It's not liek it's for the first time, it's happened before, and then disapears after a few days. but i don't know hwo to treat it - brush hardly or smoothly - or not at all at the spot?

Don't brush your gums. Pus is white fluid from an infection. Rinsing with warm saltwater may help, but it's hard to say based solely on the information provided. It comes back in the same spot? It could be a number of things... get it checked out by a real doctor.

Will try the sald trick
no doesn't always come on same spot, and it's not always white, soemtimesit's wine red

Is there anything you can get done to fix the damage?

Sorry, i missed your commen, eden...
Fillings can cover the root, so there isn't any sensitivity. For some, the root exposure is due to clenching. At rest, your teeth shouldn't be touching (they shouldn't touch very often-even when you're chewing (because food is between your teeth-preventing direct contact)). People who clench (top teeth contacting bottom teeth) can get root exposure- a nightguard, or a NTI-tss helps break the habit. Also, if you notice you are clenching, or grinding at night, and you chew gum-stop!

I read that you are a dentist graduating from Temple in the US, however you did a BS in engineering from China?
That is pretty amazing. Can you expand on this? If so I am interested in reading what made you go to China for engineering then the US for dental school. If you can post this on your blog under the heading " My career path" I would love to read it as I am sure would many others.
